Credentials for RNs and LPNs
Original Florida Nursing License
Professional resumé
Proof of one-year’s work experience
At least 2 professional references
Original BLS (must be approved by the American Heart Association)
Proof of compliance with immigration laws (I-9)
Social Security card
Valid/current driver’s license
Proof of car insurance
Proof of professional liability insurance
Proof of successful completion of a pre-employment drug test
Proof of completion of an AHCA Level II background check
Proof of health history:
Certificate of good health signed by a physician (must be within the last year)
PPD (must be within the last year) or negative chest X-ray (must be within the past 5 years)
Florida-mandated CEUs:

OSHA
